How to Enroll

New inspection facilities should contact the Help Desk to request an enrollment kit. New facilities must be NYS-DMV licensed before enrolling in the inspection program.

When you receive the enrollment kit, take time to carefully review the documents. In order to enroll, each station owner must review and execute the following documents:

  • Contract Booklet: This document includes
    • Station Participation Agreement: Outlines your responsibilities as an inspection station in the NYVIP and SGS Testcom’s responsibilities in providing equipment service and network connectivity to you for the duration of the NYVIP contract term. The NYVIP contract term is a minimum of 7 years, although you can opt out of the program at any time before the end of the agreement.
    • Purchase Order Agreement: Describes the NYVIP unit, warranties, and payment terms
    • IBM Product Acquisition Agreement Describes IBM's warranty terms for the equipment they provide.

The following forms must be completed and returned to SGS Testcom:

  • NYVIP Signature Page: Acknowledges that you have read and understand the resonsibilities and obligations contained in the Contract Booklet.
  • NYVIP Station Enrollment Form: Contact information concerning your station.
     
  • NYVIP Unit Order Form: Specifies the items and options ordered.
  • NYVIP Payment Authorization Form: Identifies how you plan to pay for the unit. Acceptable forms of payment will be as follows:

    Certified Check
    Money Order
    Credit Card (Visa or MasterCard)

If your facility does not have a credit
card, you can apply for a MasterCard
through the NYVIP program.

New stations can also transfer the inspection equipment from an existing inspection facility. Please contact the Help Desk for specific instructions on how to perform an asset transfer.
